By submitting the Online & in Person Registration Form, you acknowledge that:
  • You have been informed of the need for medical approval before joining in the fitness exercise and training program which Catherine Piot will design for you.
  • You are voluntarily choosing to participate in a physical exercise program.
  • You agree that any information, instruction or advice obtained from Catherine Piot may NOT be used as a substitute for your doctor's advice or treatment.
  • You agree that any information, instruction or advice obtained from Catherine Piot will be used at your own risk.
  • You agree to release and discharge Catherine Piot from any and all responsibilities or liabilities from injury arising from your participation in any exercise program designed by Catherine Piot on your behalf.

  • You, - The client - (Online and in Person) understands that participation in any kind of physical activity and exercise program includes risks of injury. You are responsible in providing all information concerning your health and that you are capable of participating in a training program.
  • It is YOUR responsibility to inform the trainer of any discomfort of pain during the exercise.
  • You understand that all training and nutrition programs are suggestions and are supposed to be combined with a healthy lifestyle. They are in absolutely no case intended to diagnose, replace or cure any medical conditions.
  • You acknowledge and are fully aware that Catherine Piot and Catherine Piot Institute are NOT offering any guarantees of Results. Results are individual and vary from person to person. Your Full commitment is required in order to obtain the best possible results.
  • You agree to voluntarily fill out the health and information questionnaire provided by Catherine Piot Institute and Catherine Piot and to gives honest and accurate information about your, physical condition and eating habits.
  • You are responsible to inform Catherine Piot Institute and the trainer if any change about any changes, minor or major within your health & physical condition occuring during the time of your training contract.
  • If the information you provide proved to be, incomplete, misleading, or inaccurate or false, Catherine Piot Institute and Catherine Piot will not be able to provide accurate and personalized service.
  • Catherine Piot Institute and Catherine Piot cannot be hold responsible if any form of liability, injury or even death of the client occurs.
